If you registered your child(ren) for Faith Formation in the
Trinity Cluster for the '24-'25 school year, you do not need to sign a waiver.

Cost is $30 per student entering grades 1-6 or $90 for three or more students from one family. No payment necessary for students entering grades 7-12.

Forms and payment can be dropped off at the Trinity Cluster office in Monona or placed in an envelope titled "Totus Tuus Registration" and put in the offertory basket at Mass. You can also mail forms to:
