Please download and complete one family form per family and one consent form per child. You can download forms using the links above and fill them out on your computer or tablet. (Mobile device not recommended)

Paper forms will be available soon at the St. Patrick, Monona office from 8am-12pm Monday-Thursday.

First Communion Forms - Fill out if your child is in 2nd grade

Confirmation Forms - Fill out if your child is in 10th or 11th grade.

The cost for Faith Formation is $30 per student or $85 for three or more students from the same family.

There will be no fee for sacraments this year.

Checks can be made out to "St._______ Faith Formation"  (fill in your parish name)
Payment can be dropped in the collection basket labeled "Faith Formation Registration" and please name your parish on the envelope as well.
